According to the AP:

North Carolina’s largest health insurer says higher-than-anticipated costs after two years of selling federally subsidized coverage has forced it to seek premium increases even greater than it thought would be necessary two months earlier.

The Affordable Care Act, like all government programs, is fighting for the enemy against whom it declared war. That is, we combat soaring medical costs with increasing prices in order to get promoted by demonstrating a need for more bureaucratic underlings.
