Having accidentally sent off both his birth certificate and his passport in order to renew the latter, we had to contact my son’s state of birth for a copy of the former.
The form asks for all the usual particulars of birth, and then on the line asking for the date of birth, it asks for sex and … age.
Reminds me of Sam Levinson’s comment when a form asked “Age of father, if living” … he said if living, he’d be 128 years old.
